लिंग।

पिता / पति का नाम

रोजगार / पदनाम की प्रकृति

कार्मिकों का स्थायी नाम पता।

वर्तमान पता।

रोज़गार की तारीख।

रोजगार की समाप्ति की तारीख।

काम करने वालों के हस्ताक्षर या अंगूठे का निशान।
