• Number of Workman.
• Serial Number in the Register of Workman Employed by Contractor.
• Designation/ Nature of work.
• Number of days worked.
• Wages Rate.
• Units of work done.
• Piece Rate.
• Amount of Wages Earned- (Basic Wages, Dearness allowance, Overtime, Other Cash payment(Nature of pay to be indicate). Total).
• Deductions if any, indicate nature.
• Net amount paid.
• Signature/ thumb impression of workman.
• Initials of Contractor or his Representative.
• Initials of Principal employer.

• भाग की दर.
• अर्जित आय की राशि- (मूल मजदूरी, महंगाई भत्ता, ओवरटाइम, अन्य नकद भुगतान (भुगतान करने की प्रकृति का संकेत)। कुल).
• यदि कोई हो, तो कटौती प्रकृति को इंगित करती है.
• शुद्ध राशि का भुगतान किया गया.
• काम करने वाले के हस्ताक्षर / अंगूठे का निशान.
• ठेकेदार या उसके प्रतिनिधि की शुरुआत.
• प्रमुख नियोक्ता के प्रारंभिक.
