The columns in Form A Employee register book are:

• Name,
• Gender,
• Employee Code,
• Surname,
• Date of joining the work,
• Designation or level of work,
• Nationality,
• Level of Education,
• Birth date

This Employee register Form A consists of two parts : A and B

• Part A applies to all establishments, and
  Part B applies to Mines Act

Employees in age group 14 to 18 years have to include

• nature of work,
• interval time, and
• daily hours of work in remarks column

Part B consists of fifteen fields

इस रजिस्टर में दो भाग होते हैं: A और B भाग A सभी प्रतिष्ठानों पर लागू होता है और भाग B खान अधिनियम पर लागू होता है
