• Description of the department (if applicable)
• Name of the Employee
• Date of entry into service
• Accumulation of leave (Leave due on / No. of days)
• Leave Allowed (From / To)
• Balance of Leave Carried over
• Payment for Leave made on (1st Moiety / 2nd Moiety)
• Refusal of Leave (Date of Application / Date of Refusal / Amount of Leave Refusal)
• Payment for Leave on discharge of an employee quitting employment is admissible (Date of discharge / Date / Amount Paid / Signature or left hand thumb impression of employee) 
• Signature of thumb impression of employee on receipt of Leave Book form N

• रोजगार छोड़ने वाले कर्मचारी की छुट्टी पर भुगतान स्वीकार्य है (छुट्टी की तारीख / तारीख / राशि भुगतान / हस्ताक्षर या कर्मचारी के बाएं हाथ के अंगूठे का निशान)
• लीव बुक फॉर्म एन प्राप्त होने पर कर्मचारी के अंगूठे के निशान का हस्ताक्षर
