Begin by filling in the details of the contractor at the top of the form, including the contractor’s name, registration number, and the establishment where the contract labourers are employed.

In the next section, fill in the name of each contract labourer, their designation and the number of hours work performed by them on each day.

In the next column, write the date for which attendance is being recorded.

In the next column, the contract labourers or their supervisor need to sign or provide thumbprint as proof of attendance.

The employer or authorized representative of the contractor must also sign the form to certify the accuracy of the information provided.

It’s important to keep in mind that Form D should be filled correctly and on time and also be made available for inspection by the appropriate authorities on demand as per the regulations of the Contract Labour (Regulation and Abolition) Act, 1970.
